Shoulder pain from pull-ups

Where in the shoulder?

“Shoulder pain” can mean the front crease, the top near the collarbone, the outside of the upper arm, the back near the shoulder blade, or a deep sensation that is hard to point to. Those are different descriptions, but none is a diagnosis by itself.

Front

Front crease or upper arm

Note whether it feels close to the chest, directly in the crease, or slightly down the front of the upper arm. Several structures overlap in this small area.

Top

Near the end of the collarbone

This means the small area where the collarbone meets the top of the shoulder, not the broader outer shoulder.

Outer

Over the rounded shoulder

Some shoulder sensations are felt over the deltoid or a little way down the outside of the upper arm. The felt location may not be the source.

Back

Rear shoulder or shoulder-blade edge

Separate a point at the back of the joint from a broader ache along the shoulder blade or upper back.

Deep

Inside, catching, or hard to point to

Record whether “deep” comes with clicking, catching, a sense of slipping, or weakness. Those added details matter more than guessing a label.

When does it appear?

Dead hangBefore the elbows bend, while the arms are fully overhead.
First pullAs the shoulder blades and upper arms begin to move.
MiddleWith the elbows around halfway bent.
TopNear chin-over-bar height or only when squeezing high.
LoweringDuring the descent or as the elbows straighten near the bottom.
AfterwardOn releasing the bar, later that day, at night, or the next morning.

A tender spot can help describe location, but it cannot confirm what structure is involved. This page does not include trigger-point release or other treatment instructions.

Would chin-ups or another grip change it?

Possibly, but “feels different” does not mean “fixes the problem.” The variations below change position and load-sharing. None can diagnose pain, and none is universally safer.

Pull-up

Palms away

The forearms stay pronated on a straight bar. Grip width can range from narrow to wide, adding another variable.

Chin-up

Palms toward you

The forearms stay supinated. This changes elbow-flexor and shoulder loading; it is not simply an easier pull-up.

Neutral grip

Palms face each other

The hands sit between the pull-up and chin-up positions. Overall muscle activation was broadly similar across grips in one small study.

Rings or rotating handles

Hands can turn

The forearms are not locked to one angle. More freedom of movement is a difference, not evidence that the variation treats pain.

In a small biomechanics study of healthy men, the reverse-grip chin-up produced higher proportional rotator-cuff activation than the two overhand versions tested. That is one reason not to assume that a chin-up is automatically easier on a painful shoulder.

Grip evidence: pull-up variation EMG study and shoulder-loading study. Both used small groups of healthy, mostly male participants, not people seeking care for pain.

Does a chin-up help shoulder pain?

There is no general yes. A chin-up rotates the forearm and changes shoulder-muscle loading. In a small study of 11 healthy men, reverse-grip pull-ups showed higher proportional rotator-cuff activation than the overhand versions tested. That does not mean chin-ups are harmful; it means they cannot honestly be presented as a universal shoulder-friendly fix.

The useful observation is whether the exact location, phase, and sensation change with grip. That observation still does not establish a cause.

Warning signs

Get prompt medical assessment rather than using a web page to interpret pain if any of these apply to the shoulder or arm:

  • Severe or sudden pain, especially after a fall, collision, or other injury.
  • A snap or pop followed by marked pain, bruising, swelling, or loss of strength.
  • An obvious change in shape or color, or an inability to move the arm, wrist, hand, or fingers normally.
  • Numbness, persistent pins and needles, loss of feeling, or sudden weakness.
  • Heat, redness, or major swelling, particularly with fever or feeling unwell.
